2. You must attach results from a tuberculosis test within the last calendar year.
3. You must attach a copy of your weekly class and work schedule.
4. If you have previously worked with non-human primates, you must provide CHCI with a letter of
recommendation from each facility where you worked.
5. You must attach answers to the questions listed on page 2 of this application.

On separate paper, please provide typed answers to the following questions.

  1. Briefly state your background and how you heard about the project.
  2. State your condition of health—do you have any problems that would interfere with your work, your safety, or the safety of the chimpanzees (e.g. seizures, balance problems, medications, exposure to tuberculosis, etc.).
  3. Do you have any research experience in visual observation, note taking, or using a system of abbreviations?
  4. Do you have any experience caring for primates (human or non-human) or non-primates? How do you feel about cleaning soiled areas and selflessly devoting time to another being?
  5. In a one-page or longer personal statement, indicate why you would like to work at CHCI; why you should be accepted to CHCI; what you anticipate will be your experience at CHCI; and what you hope to gain from that experience.

Please submit all portions of your completed application to the Chimpanzee and Human Communication Institute. CHCI is located on the northwest corner of the CWU campus, at the corner of D Street and Dean Nicholson Boulevard (14th^ Avenue). You may want to call 509-963-2244 before you arrive to ensure that someone will be available to take your application.
